In order to best serve our patients, we are working to become a certified Patient Centered Medical Home.
What is Patient-Centered Medical Home, or PCMH?
The Patient-Centered Medical Home model of care is defined by the National Committee for Quality Assurance as:
"a model for care provided by physician practices aimed at strengthening the physician-patient relationship by replacing episodic care based on illnesses and patient complaints with coordinated care and a long-term healing relationship."
To strengthen how we deliver services to you and maximize resources available to you, Kansas City CARE Clinic is working to attain its accreditation as a Patient Centered Medical Home.
As your Patient Centered Medical Home, we create a healthcare delivery model so that we, as your primary care physician, work with you to coordinate your treatment to ensure you receive the necessary care when and where you need it, in a way that makes sense to you.
Please note that our Patient Centered Medical Home certification will in no way impact or change how we carge or bill you. The only change you will experience with Kansas City CARE Clinic is your enhanced quality of care!
