KC CARE's Promise to You

As your medical home we believe that healthcare means caring about people, not just treating their diseases or illness.

To have a strong partnership:

We will work together to:

  • Treat your short-term illnesses
  • Manage your chronic conditions
  • Help you stay healthy

You will:

We will:

    •  Participate in making decisions about your care

    • Be open and honest about your health; tell us your symptoms and if you are seeing other doctors or taking medications we have not prescribed

    • Follow your treatment plan, or tell us why you are not following it and how we can help

    • Tell us if you disagree with our suggestions for your health
    •  Ask questions if you don't understand any part of your healthcare

    • Come to your appointments, or tell us if you can't attend

    •  Pay fees and co-pays according to  insurance and sliding fee scales

    • Be respectful of volunteers and staff

  • Provide considerate and respectful care using evidence-based guidelines from national resources

  • Communicate information about your health in ways that you understand

  • Help you arrange for care from another provided when you need it, and follow up with you after that care

  • Explain the reason for all lab work and medical tests, and what the results mean

    • Focus  our care on you as a person, not just on your symptoms or disease

    • Provide appointment times that are convenient for you as often as possible

    • Be available by phone when the office is closed

  • Explain fees for medical services

  • Review and explain your medical records

  • Give you the opportunity to have an advanced directive, which lets you have a say about how you want to be treated if you get very sick
